Service Coordination and

Case Management

The Service Coordination and Case Management program provides support to low income, disabled individuals age 18-59. If enrolled in this program, clients receive long-term case management and service coordination to ensure that their basic needs are met and living conditions are safe and appropriate.

​The types of service provided may include: General guidance with information about various agency services, set-up of homemaker / non-medical services assistance with the completion of various forms to apply for services, assistance in scheduling appointments, finding transportation, and working with other agencies to assure the needs of the client are being met.
