Housing Overview

Housing Transitions offers a continuum of housing related services for Centre County residents facing housing insecurity. Whether you need a place to stay now, in the near future, or just need to explore your options, we can help.

  • If you are currently homeless, meaning you are living in a place not meant for human habitation, on the streets or in your car, in an emergency shelter, or in a hotel paid for by a charitable or government organization, please call our Access Site at (814) 237-4865. Our on site specialist will complete an assessment to determine eligibility for housing services, referrals and resources.
     

  • Call our emergency shelter directly if you are experiencing homelessness and need a place to stay. Learn More: Emergency Shelter

How Can We Help?

I need emergency shelter now

Our Emergency Shelter program is designed to provide a safe, secure place to live while guiding residents from a housing crisis to more stable and independent living. We provide shelter and food to clients who are experiencing homelessness or are in crisis.

I need affordable housing soon

Your options are better if you contact us now! Please call if you are in an unsafe living situation or may be experiencing homelessness in the near future. We are happy to discuss options before you lose your housing.

I need support to stay in my home

For Centre County residents challenged to maintain their homes due to a disability, illness or other change in their lives, our Service Coordination and Case Management program can provide guidance. Serving individuals aged 18-59, the Case Manager connects clients with a variety of assistive services to maintain their independence and remain in their homes. Individuals must be income qualified and assessed by the case manager, who serves as a reference point, resource, and advocate for clients. 

The HOME Foundation

The HOME Foundation (THF) is a branch of Housing Transitions which provides affordable housing options for income qualified individuals and families.
 

Today, it operates as a Community Housing Development Organization (CHDO), enabling it to purchase, rehabilitate, and sell homes in the State College Borough at an affordable rate to a First Time Home Buyer. In addition, the organization owns a small portfolio of income qualified affordable rental units.

 

The organization has provided first time home buyer education, budget counseling, and post purchase counseling since the late 1990s, but as of April 2020, THF became a HUD Certified Housing Counseling Agency. The organization is part of the PHFA Housing Counseling Network and currently employs  HUD Certified Housing Counselors.

 

THF welcomes involvement from community members who wish to engage in discussion and action to find affordable housing solutions for Centre County. Regular meetings of the Board of Directors occur on the 4th Wednesday of the month at 5:30pm. The Annual Meeting of the Board of Directors is held in January of each year.
